Doctor Errors Archives

Preventing nursing errors in hospitals: part 2

A person in a Massachusetts hospital may have many more interactions with nurses than with doctors during the stay. A previous blog post discussed simple ways these medical professionals can prevent harm to patients. According to Nurse Journal, another essential nursing duty is documentation in the patient’s medical record, which often informs future decisions and prevents serious or fatal medical mistakes.

Preventing nursing errors in hospitals: part 1

People in Massachusetts may have heard of the Hippocratic Oath, which refers to ethical standards that doctors should follow. Health care providers, including nurses, are bound by legal requirements, as well. Cornell University Law School’s Legal Information Institute explains that medical professionals have a duty to their patients to adhere to the standards generally accepted in the field. Failure to meet these by acting or neglecting to act in a way that a reasonable professional would may lead to errors that are considered negligence.

Electronic health records linked to medical errors

Electronics are intended to make life easier, but the transition to electronic health records has not been smooth for many health care facilities in Massachusetts and around the United States. Indeed, recent reports indicate that in some cases, the use of electronic records has increased the incidence of doctor errors.

Medication errors in the ER

Massachusetts patients who find themselves in the emergency room may face a bewildering and overloaded system. High volumes of patients, staffing cutbacks and the pressures of providing care in a stressful environment mean that medication errors occur disproportionately in ERs around the United States.

Informed consent: A must for all patients

Informed consent is a crucial component in Massachusetts doctors’ relationships with their patients. In fact, the code of ethics for health care providers demands that doctors and nurses make every effort to ensure that their patients understand their potential plans of treatment as well as any other options that may be available. This is true even when patients lack the ability to make their own decisions.

What are never events in health care?

If you are like many people in Massachusetts, you are very concerned about the quality of your health care. You may understandably be a bit worried about the possibility of a medical error happening. Sadly, these events can and do happen every day. Many serious injuries or even deaths have resulted from the negligence of or lack of proper care by medical professionals. When looking at medical errors, it is important to understand what types of things may be considered errors and how they are classified.

Bariatric surgery: Patients should be warned about vision loss

Doctors in Massachusetts give their clients a lot of advice and suggestions when it comes to managing their weight. One suggestion is to undergo a weight loss surgery, but it is important for doctors to make sure that their patients are aware of all the risks that can happen, especially the loss of vision.
